TOGETHER THAN WE ARE ALONE
To participate on a team a student must turn in a MSHSAA
Physical form. If you
had a physical after
February 1, 2012 will be good for the entire 2012-2013 school
year.
If you had a physical prior
to February 1, 2012, you will need to have another one before
participating in any sport during the 2012-2013 school year.
There are five parts to the new form.
1.
HISTORY FORM:
This portion needs to be completed by the parent prior to
the student’s physical examination and brought to the visit.
2.
PHYSICAL FORM:
This needs to be signed by the medical professional
performing the physical.
Make sure it’s signed in the appropriate box at the
bottom of the page.
3.
STUDENT AGREEMENT: Please review with your
student the MSHSAA
Eligibility Brochure before signing.
4.
PARENT PERMISSION: Parent must provide
insurance information (insurance company and ID number) and sign

PARENT AND STUDENT SIGNATURE (CONCUSSION FORM): Both student and parent
signs after receiving and reading the Concussion Packet.
If your student-athlete does not have all portions of this
form completed, they will not be able to participate until all
parts of the form are complete.
