PLEASE NOTE: If your child texts or calls you during the school day complaining of illness, please encourage him/her to see the nurse. The nurse's office is located in the Elementary School building across from the school office. She is on duty 7:30 a.m. to 3:30 p.m. during the school year.

2017 – 2018 REQUIRED IMMUNIZATION FOR INCOMING 8TH AND 12TH GRADE STUDENTS
Students entering 8th and 12th grades are required to have the Meningococcal vaccination.
GRADE 8 students must have one (1) dose of Meningococcal vaccine (MCV).
GRADE 12 students must have two (2) doses of the Meningococcal vaccine (MCV).
NOTE: The exception for 12th grade students - if they received one (1) dose after age 16. In that case only one (1) dose would be all that is required.
Contact your doctor now to see if your child has already received the MCV vaccination (if not, please schedule an appointment). Have the doctor’s office fax your student’s MCV vaccine verification to 636.225.4518 or e-mail to firstname.lastname@example.org. You may also drop a copy off at the school office.
This immunization record MUST be turned in before the first day of school on August 16, 2017.
