PLEASE NOTE: If your child texts or calls you during the school day complaining of illness, please encourage him/her to see the nurse. The nurse's office is located in the Elementary School building across from the school office. She is on duty 7:30 a.m. to 3:30 p.m. during the school year.

2021-2022 REQUIRED IMMUNIZATION FOR 8th, 9th, 10th and 12th GRADE STUDENT
Students entering 8th, 9th, 10th and 12th grade are required to have the Meningococcal vaccination(s).
8th, 9th and 10th grade student:
MUST have one (1) dose of the Meningococcal vaccine (MCV).
Note: Dose must be given after 10 years of age.
12th grade students:
Two (2) doses of Meningococcal vaccine (MCV) are required.
EXCEPTION: If 12th grade student received one (1) dose after age 16. In that case, only one (1) dose that is required.
Contact your doctor now to see if your child has already received the MCV vaccination (if not, please schedule an appointment). Have the doctor’s office fax your student’s MCV vaccine verification to 636.923.3656 or e-mail to email@example.com. You may also drop a copy off at the school office.
REMINDER: This immunization record MUST be turned in before the first day of school on August 24, 2021.
