Required immunization for 2016-2017 School Year - Incoming 8th and 12th Grade Students
I want to inform you about a change in immunization requirements for the 2016-2017 school year now so you can make a doctor's appointment as soon as possible and avoid the summer rush.
Students entering 8th and 12th grades will be required to have Meningococcal vaccinations.
Grade 8 students will need one (1) dose of Meningococcal vaccine (MCV).
Grade 12 students will need two (2) doses of the Meningococcal vaccine (MCV). The exception to this is if the student received one (1) dose after age 16. In that case that one dose would be all that is required.
Contact your doctor now to see if your child has already received the MCV vaccination. The doctor can fax your student’s MCV vaccine verification to 636.225.4518 or e-mail the information to firstname.lastname@example.org. You may bring a copy of the shot record to school if that is convenient.
This immunization record MUST be turned in before the first day of school on August 16, 2016.
